My guess is that Trump doesn't think or care how his words will be interpreted. He's such a narcissist he things everything he says will be worshipped. He has no sense of shame--literally, none, another characteristic of a hard narcissist.
Well, that helps explain the childish behaviours and outbursts. Problem solved.
