Medicare is a fee for service system. No one has any incentive to avoid cost. Doctors, drug companies, hospitals think up ever increasing costly procedures and the patients have no incentive to refuse anything.

During the healthcare debate, GOP congressmen continuously quoted "United for Health" (I think that's the correct name), which is United Healthcare Inc., as if it were an independent think tank.
